my motivation to study has severely declined. my friend and i borrowed a library book which was a picture test book, with photographs of weird rashes, patients so we could do "spotters" as in spot diagnoses. so, we started. there was a picture of a man's face. tried REALLY hard to just inspect every square inch, but he looked perfectly normal. i thought, "am i missing something?" and i said, " he looks normal, if a bit fat". turned out, the answer was bilateral parotid gland swelling. what i mistook for chubby cheeks was actually not one but TWO salivary glands all swollen up.
next picture. this picture was of a man's shoulders and abdomen. i looked at every square inch. "please, this is just another fat patient", i said. turned out, he has cushingoid features, a hormonal disease which either means excess steroid treatment or a tumour somewhere producing steroids in the body.
somehow i don't think endocrinology is going to be my strong suit. i'm so scared now to call people fat, i think the politically correct term now is pleasingly plump or "he has a gland problem".
